Just logged in to see if anyone was about and goodness me, this thread brings back some memories. I'm actually tearing up a little bit reading Mike's posts 
(RIP Mike - gone but not forgotten).
Anyway, hope everyone is keeping well. Remember: be nice to each other, life is too short.

I did indeed! While I have since taken down the Wordpress blog I made for it, you can still download the pdf:

That revised pdf is from 2013. The original was written in (wait for it...) 2007. That's almost as old as some of my students!
[And yeah, I'm not sure what's better: writing a guide, or turning up #11 on Ste_Js list. I'm thinking it might be the list haha. ]
